  • Setting dietary goals to improve lifestyle.
  • Keeping a food diary to make changes more realistic.
  • Consultation: 60 minutes.


Modifying and individualising diet to suit lifestyle will be another 45 – 60 minutes.

Remember this is an interactive consultation and questions are encouraged for you to understand your condition fully.

First Follow-Up Assessment:

  • Physical measurements.
  • Receive individualised meal plan, if requested by patient.
  • Receive a 5-day cycle menu.
  • Assessing food diary to identify problems.
  • Dietary goals revisited.
  • Consultation between 30 – 40 minutes.

Follow-Up Assessments:

  • Physical measurements.
  • Variety of topics to be discussed.
  • Consultation: 20 – 30 minutes.

Topics may include appropriate recipes, shopping lists, eating away from home, holidays and travelling, how to change your favourite recipe, label reading and so much more. You, as the patient,may also suggest topics related to your specific need.

12 Week Lifestyle Program:

What to expect from this 12 week program:

  • An initial assessment of 60 minutes
  • Once a month a 45 minute check-in session
  • Three times per month a weigh-in session of 20 minutes.




Follow-Up appointments are essential in keeping individuals motivated to make the necessary lifestyle changes and to assist in finding solutions to problems and barriers experienced. This will ultimately contribute to your success and a better lifestyle.
